Tesamorelin

Overview

Tesamorelin is a synthetic analog of growth hormone-releasing hormone (GHRH) consisting of the full 44 amino acids of native GHRH with a trans-3-hexenoic acid modification at the N-terminus. This modification enhances the peptide's metabolic stability while preserving its full biological activity at the GHRH receptor.

Originally developed and extensively studied for its effects on visceral adipose tissue, Tesamorelin has been the subject of numerous clinical trials and represents one of the most thoroughly characterized GHRH analogs in research literature.

Tesamorelin's mechanism of stimulating physiological pulsatile GH release, rather than providing exogenous GH, means it works within the body's natural feedback systems — somatostatin-mediated negative feedback remains intact, preventing supraphysiological GH levels.

Tesamorelin activates the GHRH receptor on anterior pituitary somatotroph cells with similar potency to native GHRH(1-44). The trans-3-hexenoic acid modification at the N-terminus protects against DPP-IV cleavage, the primary route of GHRH degradation, while the full 44-amino acid sequence preserves all receptor-binding epitopes.

Upon GHRH receptor activation, the Gs/adenylyl cyclase/cAMP cascade stimulates both immediate GH release from preformed vesicles and transcriptional upregulation of GH gene expression for sustained production. Importantly, Tesamorelin's pulsatile stimulation pattern preserves the somatostatin feedback loop, maintaining physiological GH oscillation.

The resulting GH elevation activates hepatic IGF-1 production, which mediates many of the downstream metabolic effects including enhanced lipolysis in visceral adipose tissue, improved insulin sensitivity, and modulation of lipid metabolism.

Handling & Reconstitution

1. 1Allow the vial and sterile water to reach room temperature (15–25°C).
2. 2Using a sterile syringe, withdraw the appropriate volume of sterile water.
3. 3Inject the water slowly along the inside wall of the vial — do not spray directly onto the powder.
4. 4Gently swirl the vial until the powder is fully dissolved. Do not shake vigorously.
5. 5Once reconstituted, store at 2°C – 8°C. Protect from light.
